Families: children under 18 are $49 per month with at least one enrolled adult, and kids share the access level of their parents' tier. Two parents on Platinum means the whole household, kids included, gets same-day scheduling and after-hours physician access. If a parent is on Gold or not enrolled, children have Gold-level access.
Scheduling subject to physician availability. *Diamond's friends and family benefit covers one-off acute visits for guests from out of state who would otherwise head to urgent care or the ER during their short stay. It isn't ongoing primary care for non-members.

How do I know which tier is right for me?
The honest answer: that's what the free meet and greet is for. Different households use primary care differently, and we'd rather match you with the right tier than oversell you on one you'll never use.
As a rough guide: Gold suits adults who want a dedicated primary care physician but only need occasional visits, and adults under 40 get Gold at a reduced $49 rate. Platinum, our most popular tier, works for adults who want unlimited visits and direct cell phone access for urgent issues. Diamond is our concierge tier for patients who want full healthcare system navigation and the broadest access.
Can I change tiers later?
Yes, within reason. Members can move between tiers as their needs change over time. What we don't allow is short-term tier-hopping, like upgrading to Platinum because you came down with something and dropping back to Gold once you feel better. The smaller panels and direct access at higher tiers depend on members at that level being committed to it, so tier changes are meant to reflect a real shift in how you use primary care, not a temporary one.
What's the catch with the annual discount?
None. Pay for the year upfront and we discount the price by 10%. If you cancel mid-year, we prorate the refund based on the months you didn't use.
Can I use my HSA or FSA for the membership?
HSAs, yes. Recent federal legislation made HSA funds eligible for Direct Primary Care memberships, within monthly limits set by federal law. Gold and Platinum fall within those limits in full. For Diamond, we'll walk you through exactly how HSA payment works at your meet and greet. Bring your specific HSA plan details and we'll sort it out together.
FSAs depend on your plan administrator. Some allow DPC memberships, others don't. Bring those plan details too and we'll help sort it out.
What about my kids? How does family enrollment work?
Children under 18 are $49 per month and can enroll when at least one adult in the household is also a member.
A child's level of access matches the lowest tier among the eligible adults in the family. So if both parents are on Platinum or above, the child has VIP cell phone access too. If one parent is on Gold or isn't enrolled, the child gets Gold-level access. The rule keeps family-wide concierge benefits tied to the family being all-in.
The simplest way to think about it: kids inherit the access of whichever adult in the family has the least access.
Do I still need health insurance with a Simplicity Health membership?
Yes, we recommend it. The membership covers your everyday primary care. Health insurance still matters for the things outside our scope: hospitalizations, ER visits, specialists, surgeries, and major medical events. Many members carry a high-deductible plan alongside their membership. See how the pairing works →
Do you have availability for new patients?
Yes. We keep individual panels small on purpose, so spots are real and finite. We're actively bringing on additional board-certified physicians to keep up with patient demand, but once a doctor's panel fills, that doctor closes to new patients and a rolling waitlist begins.

What if I'm not a member but want to see one of your doctors?
We see non-members for one-off consultations at $300. If you decide to enroll as a Simplicity Health member within a year of that visit, the $300 is credited toward your membership cost.
